A young Queen Charlotte assists Reynolds and Brimsley in washing up George at the start of Queen Charlotte episode 4. She then demands to know about her husband's condition.

George is present on his farm as we go back in time. He is still blissfully unaware of his impending wedding at this point. George had a fit that almost renders him catatonic after learning of his marriage to Charlotte from his mother. One week before his wedding, the King is still healing. The royal doctors try everything, including bleeding him and using tourniquets and poison bug stings. But nothing seems to be working. Princess Augusta wants to assist her child and seeks out Doctor John Monro, a specialist in treating nervous system disorders.

Upon consultation, the doctor suggests that speaking with the King may be a potential remedy for the medical condition. At first, it appears to have a positive effect, as Monro convinces George to prepare for his wedding through assertive communication.

As the story progresses, we discover that George has fallen in love with Charlotte, but chooses to live separately from her to protect her from his affliction. He successfully avoids her until she voluntarily confronts him inside the conservatory. To be with his new Queen, he then swears that he would do everything it takes to become well. Because George sought a long-term cure for his lunacy, he allowed Monro to employ an ineffective therapeutic approach. Monro's excellent strategy involves torturing the King into ceasing the fits. His concept of rehabilitation consists of really painful shaving sessions, followed by cold baths, and torture chair time. But as time goes on, George gets sick of the doctor's harsh methods, and his desire to get to know his new Queen grows greater.

One day, as a sign of goodwill, George delivers Charlotte Monro's lap dog, affectionately known as "the deformed bunny" by Charlotte. Monro tries to oppose when George and his wife finally moved into Buckingham House, but the King doesn't listen. Just before the triumphant Danbury Ball, as George and Charlotte's relationship deepens, George decides to permanently terminate the doctor.

After the ball, the King walks into the kitchen and finds Monro. Some preparation is done on the stove. He tries to explain that the doctor's assistance is no longer necessary, but Monro suddenly admits it. He discloses to the King that he is presently employed by the Queen. Later, he makes known the Queen's pregnancy, which is what led to the most recent outburst from the King. George awakens the following morning to discover Charlotte leaving to meet Princess Augusta. The Queen rudely interrupts the Princess and confronts her. Charlotte is furious because she is not aware of her husband's insanity. On the other side, Augusta is enraged by Charlotte's lack of respect for her role as the Queen. Unfortunately, George, who was trailing his companion, overhears the argument. He feels hurt and finds his way back to Doctor Monro and his brutal method of treatment.
